Mother of Doris Jean Trainor Cozzens (Eugene “Jim” Jr.) and Milton Martin Trainor, Jr. (Lisa).

Grandmother to Christopher Anthony Cozzens (Shantell), Stephanie Frances Trainor, Kelly Lynne Cozzens Keller (Chad), Stacey Marie Trainor, Stephen Anthony Trainor. Great-Grandmother to Whitney Nicole Martin, Tyler Curtis Keller, Allie Lynn Cozzens, and Julia Grace Keller. Predeceased by her parents Kenneth Keith Addison and Thelma Lizette Hauser. Sister of Doris Mae Addison St. Amand and predeceased by Kenneth Joseph Addison and her Uncle Joseph “Joe” Hauser.

Also survived by many other relatives and friends.

Hazel was a passionate dancer who Lived to Dance. She lived her life the way she wanted that is for certain. She started the New Orleans Jamaica Whip Dance club c1984 which then became the New Orleans Jamaica Dance Club. Hazel was very adventurous and followed her love of dance to California in 1985 when she was 57 years old because she could not find enough dancers in the New Orleans area. She formed and supported many dance clubs there. She lived there until 2009 when she returned to New Orleans.

It is with sincere and very deep appreciation that the family would like to thank Wynhoven Healthcare and Notre Dame Hospice. The care and love shown by both organizations surpasses even the love of family.

Family and friends are invited to attend a Memorial Service at NOLA Spaces/Dance Quarter, 1719 Toledano St., New Orleans, LA 70115 on Saturday, August 25, 2018 from 3:30 PM until 6:00 PM.
